In an effort to help guide the success of completing judicial imposed sanctions, the Office of Judicial Affairs has published numerous documents, forms, and informational packets for students. These published documents can either be picked up in the Judicial Affairs Office in Wood Support Services, Room 240, or by downloading the forms below.

Academic Integrity Workshop
Academic integrity is an essential component to academic success and ethics. Students who want to know more about the Academic Misconduct Policy are encouraged to visit the plagiarism web page and review the excellent resources there. In addition, students may take a free Academic Integrity Workshop by contacting the Judicial Office.
Office of Community Service
Student Judicial Forms/Documents
Community Restitution Verification Form: used for verification that a student has completed their community restitution hours at a judicially sanctioned/approved agency. The form requires the signature of both the student and volunteer coordinator of the agency where the restitution hours were completed.
Note: The community restitution requirement due date is determined by the Director of Judicial Affairs.
