Forms and Documents
Academic Misconduct Report Form: A faculty member who has determined that academic misconduct has occurred in a course at Eastern is required to document the incident, meet with the responsible student(s), and inform the student(s) of the sanctions imposed. Faculty members who impose a sanction less than a failing grade for the course are encouraged to submit an Academic Misconduct Report form and supporting material to the Director of Judicial Affairs.
