*All teacher candidates admitted by CARE after April 2013 are required to submit the Pre-student teaching application and the Student teaching application using their TK20 Account.  All other applicants can submit their application to the office of Educational and Clinical Studies.

The STUDENT TEACHING APPLICATION is submitted one semester prior to the actual student teaching experience and is due the first week of the fall semester and the first week of spring semester. A complete application includes a signed cover letter, resume, unofficial transcripts, proof of registration for PRAXIS II and the signature of the candidate’s Education advisor.

The EVALUATION RUBRIC is an assessment tool completed online by the university supervisor, cooperating teacher and teacher candidate at the midterm and final. This evaluation requires specific feedback in order to support the candidate’s growth as a student teacher and future professional.

The RATING GRID is a compilation of the ratings from the teacher candidate, cooperating teacher and university supervisor. This summative form is submitted at the midterm and final. These ratings take into consideration the lesson plans, observation reflections, observation reports and any significant documentation related to the candidate’s performance and professionalism during student teaching.

The OBSERVATION REPORT is a form completed by the university supervisor following each formal observation or conference with the teacher candidate. This is a summary of the observed lesson(s) with recommendations for improvement. A minimum of seven reports are included as part of the formative evaluation for a student teacher candidate.

Common Forms

The LESSON PLANtemplate is recognized by the Education Unit as an essential tool during student teaching. During student teaching all lessons must be completed using this form. The intent is to develop effective planning skills as a candidate progresses during student teaching. This comprehensive format includes lesson development, methodology, evaluation, differentiation and connections to state and national curricula standards.

The OBSERVATION REFLECTION is a series of questions for the teacher candidate to reflect upon and consider during the post-conference with the cooperating teacher and university supervisor. This form assists in developing teacher candidates who participate in meaningful reflection that results in improved teaching techniques.
