If you are taking courses during the winter session and/or the spring 2015 semester and you would like to request academic accommodations, please submit an online Service Request Form as soon as you are able. Here are the links to the Service Request Form and the Audio Text Request Form:
Please note that accommodations do not roll over; you need to request your accommodations each semester. After you submit the Service Request Form, an OAS staff member will contact you on your Eastern email account regarding your Letters of Accommodations. Please note that an appointment is needed to pick up your letters.
